How to File Your Income Tax (Houseman)

1. Log in to e-Penyata Gaji to download your Penyata Pendapatan Tahunan. After logging in, click "Penyata Pendapatan Tahunan" on the left column, and download the file "EC DEC 201_".

If this is your first time filing your income tax, you will first have to register for an income tax number:
2. Go to LHDN website. Move your cursor to ezHASiL on the top row and choose e-Daftar in the dropdown menu.
3. Download Panduan Mengisi Borang and Panduan Muat Naik Dokumen on the left column and read those instructions.
4. Follow the instructions on the e-Daftar webpage and fill up your personal information in the borang pendaftaran online.

  • No. Majikan KKM is E292072802
  • Then you have to upload the front of your IC through one of the following methods:
    • By email: edaftar@hasil.gov.my
    • By fax: 03-42894020, 03-42894080, 03-42893737
    • By the file uploader on the website (but this is tricky since you need to upload a .GIF file with the specific size of 40-60Kb)
      • This is how I did it:
      • I cropped off the empty portions of my scanned file around my IC using Windows Live Photo Gallery's Edit option.
      • I downloaded GIMP (GNU Image Manipulation Program) from the Internet (just search in Google).
      • I opened the image file in GIMP, clicked "Image"-->"Scale Image..." on the top row.
      • I scaled down the image to a smaller arbitrary value, saved the file as ".GIF" and checked the file size of the saved file.
      • I repeated the above step until I obtained a file with the size of 40-60Kb.
      • (Is there an easier way? I don't think a lay person is expected to know this!)
5. Wait 3 days then go to Semak Status Individu to track your application process. Your Tax Reference No. will be provided in the form of SG xxxxxxxxxxxx (2 alphabets + 12 digits). Your e-Filing PIN No. will also be provided in the form of xxxx-xxxx-xxxx-xxxx (16 digits). (Initially they did not provide me with the e-Filing PIN No. so I emailed them to request for one, and then it was provided a few days later on the same webpage.)

6. Go to LHDN website. Move your cursor to ezHASiL on the top row and choose e-Filing in the dropdown menu. Change the language to ENGLISH at the top right corner if you need to.
7. On the left column on the e-Filing website, click First Time Login. Key in your e-Filing PIN No.
8. After that, just follow all the instructions. They are fairly straightforward. Of note:
9. Penggajian on your e-BE form = Jumlah Gaji, Bayaran and Elaun-elaun on your Penyata Pendapatan Tahunan
10. Remember to include the following amounts in the relevent areas on your e-BE form:
Potongan Cukai Pendapatan on your Penyata Pendapatan Tahunan,
and Caruman kepada KWSP on your Penyata Pendapatan Tahunan,
so that you do not pay extra.

Hospital Kajang Laboratory Tests 2011

Recently my HOD assigned some people to summarize which tests are done with which bottles and take how long. With reference to the Hospital Kajang Laboratory and Pathology handbook and after discussing with some lab personnel, here is the brief summary of Hospital Kajang Laboratory Tests 2011 (alternate link). To all Medical HOs, please take note of which tests require a written clinical history (including which are the main points for the clinical history) and which tests require specialist's signature (especially Trop T and D-dimer). Please point out any errors too, thank you.

Streamyx to Unifi

When converting from Streamyx TM Net to UniFi TM Net (high speed internet), one must be careful to perform the following in order to avoid paying double bills:

1. Apply for UniFi TM Net and follow through all the procedures.

  • Please make sure you have a 4-plug extension wire and an internet cable 4-10m in length (depending on the distance between your TV set and desktop computer) during the time of installation to facilitate efficiency.
  • Note that your telephone number will be changed to a new number you chose during application.
  • During installation of UniFi, your old telephone number line and old Streamyx broadband service will be "physically" terminated. In other words, although you will not be able to use your old telephone number line and old Streamyx broadband service anymore, YOU MAY CONTINUE TO BE CHARGED UNTIL YOU APPLY FOR THE TERMINATION OF THOSE LINES.
2. Apply for the termination of your old telephone line, your 1525 line (if you had dial-up internet with your old telephone line before) and your old Streamyx broadband account. Call TM for the details. 
3. Bring along 2 copies of your IC, your old telephone set, your old Streamyx broadband modem and fill up the termination application forms for the above 3 lines/accounts at a TM Point.
4. Keep the termination slip for the above procedures.
5. Some of the bills of the terminated services may continue to arrive for a short period of time. Do not pay and wait for the last pink bill to arrive. Then bring along the unpaid bills and the termination slip to a TM Point to apply for a waiver of the bills.

I am writing this because a friend of mine paid double bills for a few months, not knowing concerning the above. I hope this will be useful to hopeful UniFi users and applicants. Suggestions are welcome.

Hospital Kajang Drug List 2011

Hospital Kajang Drug List (alternate link) in the making...

I am so used to using NTUH's pocket and online drug formulary that I had to have a Hospital Kajang Drug List in my pocket, modified from SGH HO Druglist (http://www.geraldtan.com/doctors/index.html), Hospital Kajang Drug Formulary, Kementerian Kesihatan Drug Formulary and MIMS Malaysia.
This is purely for convenient reference; the author is trying his best and disclaims all responsibility for any errors or omissions or for the results obtained from use of the information contained in this work.
